Everything you need to know before your first visit — what to bring, how to submit paperwork, and what to expect when you arrive.

Getting Started

Referral Required

A referral from your primary care physician or a qualified specialist is required for all new patient consultations. Your referring provider should send the referral directly to our office prior to your visit — you do not need to bring it with you. If you’re unsure whether your referral has been received, give us a call and we’ll help sort it out.

What to Bring
  • Photo ID and insurance card — health insurance is required; we do not offer cash-pay services
  • Copies of relevant medical records related to your pain condition, including any prior pain management notes
  • A complete list of your current medications, including over-the-counter drugs and supplements
  • A list of prior treatments you have tried — physical therapy, chiropractic care, injections, or any surgeries
  • Any relevant imaging or diagnostic studies (MRI, CT, X-ray) — a printed or digital report is preferred, as we are not always able to access disc or digital image files

Plan to arrive 15–20 minutes early if you haven’t completed your paperwork in advance.

New patient consultations typically take between 45 minutes and an hour. We want to take the time to thoroughly understand your history, review any imaging, and discuss your treatment options — so please plan accordingly. Completing your paperwork before you arrive will help things move faster.
Your first visit is a consultation — not a procedure. You’ll meet with the physician to discuss your pain history, symptoms, previous treatments, and goals. We’ll review any imaging or records you bring, perform a physical exam, and work together on a treatment plan. You won’t leave without a clear next step.
Records can be emailed directly to medicalrecords@northfloridapainspecialists.com or your provider’s office can fax them prior to your visit. For imaging, a printed or digital report is preferred — we are not always able to access disc or digital image files.
We ask that cancellations be made at least 48 hours in advance so we can offer that time to another patient in need. A no-show fee applies to any appointment cancelled within 48 hours of the scheduled visit, as well as to any no-show.
