Send referral documentation and records to our referral inbox. Please do not email protected health information unless using a secure system.

What to Include with Your Referral

Complete referrals move faster. Here’s what helps us hit the ground running at the first visit.

Patient Demographics & InsuranceFull name, DOB, contact information, and current insurance coverage.
Reason for Referral / Clinical QuestionWhat you’d like us to evaluate or address — the more specific, the more useful our report back to you.
Relevant ImagingMRI, CT, and X-ray reports. Ideally imaging discs as well — especially for spine and neuromodulation candidates.
Prior Treatments & OutcomesWhat has been tried, for how long, and with what effect. This includes prior injections, therapy, and any prior procedures.
Current Medication ListAll medications, including dosages. Particularly important for patients on controlled substances.
Any Urgent ConsiderationsFlag progressive neurological deficits, active infection, coagulopathy, or any concerns requiring expedited scheduling.

Comprehensive Evaluation

Full history, physical examination, review of prior treatments and outcomes, and assessment of relevant diagnostic labs, imaging, and other studies.

Individualized Treatment Plan

We present the best available treatment options for each patient’s specific condition, then work collaboratively with the patient to pursue the approach that aligns with their goals and preferences.

Consult Note to Your Office

A written consult note is sent to the referring provider following the initial evaluation, summarizing our findings, clinical impression, and treatment plan.

Procedural Authorization & Scheduling

All procedural intervention authorizations are handled by our team. Depending on the patient’s preferences and clinical needs, procedures can be performed in a clinic-based setting, an outpatient ambulatory surgery center, or a hospital setting.

Collaborative care: We work closely with other specialists to provide the best possible outcomes for our patients.

Insurance Accepted

We participate with most major commercial plans, Medicare, and Florida Medicaid. Contact us if you don’t see a patient’s plan listed.
