New Patient Information
Everything you need to know before your first visit — what to bring, how to submit paperwork, and what to expect when you arrive.
Referral Required
A referral from your primary care physician or a qualified specialist is required for all new patient consultations. Your referring provider should send the referral directly to our office prior to your visit — you do not need to bring it with you. If you’re unsure whether your referral has been received, give us a call and we’ll help sort it out.
- Photo ID and insurance card — health insurance is required; we do not offer cash-pay services
- Copies of relevant medical records related to your pain condition, including any prior pain management notes
- A complete list of your current medications, including over-the-counter drugs and supplements
- A list of prior treatments you have tried — physical therapy, chiropractic care, injections, or any surgeries
- Any relevant imaging or diagnostic studies (MRI, CT, X-ray) — a printed or digital report is preferred, as we are not always able to access disc or digital image files
Plan to arrive 15–20 minutes early if you haven’t completed your paperwork in advance.
