Failed Neck Surgery Syndrome (FNSS)

Persistent pain after cervical spine surgery is a recognized clinical condition — not an indication that the surgery failed. For many patients, the pain mechanism outlasts the structural problem that prompted surgery, and effective interventional options remain available.

Overview

What Is Failed Neck Surgery Syndrome?

Failed neck surgery syndrome describes the experience of patients who continue to live with significant neck pain, arm pain, or neurological symptoms after cervical spine surgery — including anterior cervical discectomy and fusion (ACDF), cervical disc replacement, or laminectomy. It is a recognized clinical condition that reflects the complexity of nerve pain rather than a failure of surgical technique.

Nerve roots that were compressed for an extended period before surgery may not fully recover even after decompression. Adjacent segment disease — degeneration at levels above or below a fusion — can produce new or recurring symptoms over time. Epidural scar tissue can re-tether nerve roots after surgery. In some patients, sensitization of the nervous system predated the surgery, meaning the pain mechanism itself requires treatment beyond structural correction.

Treatment

How We Approach Failed Neck Surgery Syndrome

Cervical epidural steroid injections address ongoing nerve root inflammation in patients with residual radicular symptoms. Cervical medial branch blocks and radiofrequency ablation address the facet joint component — which frequently becomes more prominent after fusion surgery alters load distribution to adjacent levels.

Spinal cord stimulation is one of the most effective longer-term options for patients with persistent neuropathic pain after cervical surgery — particularly arm pain that has not responded to injections and medications. A thorough evaluation of your surgical history, current imaging, and pain pattern is the right starting point to determine which approach is most appropriate.
