Intercostal Neuralgia (Chest Wall Nerve Pain)

Intercostal neuralgia produces burning, stabbing, or band-like pain along the chest wall that follows the path of an irritated or damaged rib nerve — a condition that responds well to targeted nerve blocks and comprehensive nerve pain management.

Overview

What Is Intercostal Neuralgia?

The intercostal nerves run along the undersurface of each rib, traveling from the thoracic spine around the chest wall and supplying sensation to the skin and muscles between and around the ribs. When these nerves are damaged, compressed, or irritated, they produce burning, stabbing, or aching pain that typically wraps around the chest in a band-like pattern, following the course of the affected rib or ribs.

Common causes include postherpetic neuralgia from shingles, prior thoracic surgery, rib fractures, chest wall trauma, and nerve entrapment from scar tissue. The pain can be severe and can significantly affect breathing, movement, and sleep. Because chest wall pain can overlap with other causes of chest discomfort, an accurate diagnosis is an important first step before beginning interventional treatment.

Treatment

How We Treat Intercostal Neuralgia

Medication management targeting nerve pain is typically the foundation of treatment. Intercostal nerve blocks are a key interventional tool — confirming the diagnosis and providing therapeutic relief by delivering medication directly along the affected nerve. A series of nerve blocks is generally more effective than a single injection and can interrupt the pain cycle over time.

For patients with severe or refractory intercostal neuralgia that has not responded adequately to medications and nerve blocks, peripheral nerve stimulation targeting the intercostal nerves may be appropriate. A consultation will help determine the most appropriate treatment pathway for your specific pattern of pain.
