Scoliosis is an abnormal lateral curvature of the spine that can affect patients from childhood or develop in adulthood as a consequence of degenerative changes. While not all scoliosis causes pain, degenerative scoliosis in adults frequently produces significant chronic pain through several mechanisms that are directly addressable with interventional treatment.

The abnormal spinal curvature creates asymmetric loading of the facet joints and intervertebral discs — accelerating degeneration on the concave side of the curve. Foraminal narrowing on the concave side can compress nerve roots, producing radicular symptoms into the arms or legs. The paraspinal muscles work continuously to compensate for the altered alignment, generating chronic myofascial fatigue and pain. In many patients, all of these mechanisms are present simultaneously and each requires its own treatment approach.

How We Treat Scoliosis-Related Pain

Facet joint injections and medial branch blocks with radiofrequency ablation address the asymmetric joint loading that drives much of the axial pain in scoliosis. Epidural steroid injections target nerve root compression at the levels of foraminal narrowing. Trigger point injections address the substantial myofascial burden that builds up in compensating paraspinal muscles.

For patients with significant neurogenic claudication from scoliosis-related lumbar stenosis, the MILD procedure or spinal cord stimulation may be considered.
