Trochanteric Bursitis (Outer Hip Pain)
Outer hip pain that aches on lying on your side, worsens with prolonged walking, or radiates down the outer thigh is often trochanteric bursitis — a common and highly treatable source of chronic hip pain.
What Is Trochanteric Bursitis?
The greater trochanter is the bony prominence on the outer side of the hip where several muscles and tendons attach. A fluid-filled cushioning sac called a bursa sits between this bony prominence and the overlying soft tissue. When this bursa becomes inflamed — from repetitive friction, muscle imbalance, trauma, or increased loading — it causes the persistent aching or burning lateral hip pain that characterizes trochanteric bursitis.
The condition is now more broadly referred to as greater trochanteric pain syndrome, recognizing that in many patients the pain also involves the gluteal tendons that attach nearby. It is more common in women and in middle-aged to older adults, and is frequently associated with underlying hip or lumbar spine pathology, leg length discrepancy, or hip muscle weakness. It can be a persistent and functionally limiting condition that significantly disrupts sleep and activity.
How We Treat Trochanteric Bursitis
A corticosteroid injection into the trochanteric bursa is one of the most effective short-to-medium term treatments — quickly reducing inflammation and providing significant relief in most patients. The injection is performed under image guidance to ensure accurate placement. Platelet-rich plasma (PRP) injections may be appropriate for patients whose symptoms recur after corticosteroid treatment, targeting the underlying tendon pathology.
Physical therapy focused on hip abductor strengthening, iliotibial band flexibility, and movement pattern correction is important for longer-term management and reducing the likelihood of recurrence. Addressing any contributing factors — including spine pathology, hip joint arthritis, or leg length discrepancy — is part of a comprehensive approach. A consultation will determine the most appropriate treatment plan for your specific situation.
